universal medication form
The Tennessee Pharmacists Association has developed a universal medication form for hospitals and other providers to use to assist with medication reconciliation.
The Tennessee Nurses Association was a partner in this statewide effort to improve medication safety in Tennessee by providing a universally consistent process for communicating vital patient information regarding medications and allergies across the continuum of care. Patients should be encouraged to carry an up-to-date medication and allergy list with them at all times to be available at entry and exit from all health care encounters.
The Tennessee Hospital Association's quality committee has approved the form. Hospitals are encouraged to route this information to their medical staff members, distribute them to patients, nurses and other staff members, and make them available in waiting rooms.
